`entries.evidence.prepare`

Prepare an immutable supporting-original upload for an active editable transaction. Supply entryId, filename, mimeType, sizeBytes, sha256 and a stable idempotencyKey. Skip PUT when alreadyUploaded, then confirm. Adds documents alongside existing originals without creating purchases or changing financial fields.
